Nutrition

Nutrition services are a cornerstone of preventive and therapeutic care, reaching individuals throughout the entire care continuum—from acute inpatient environments to long-term residential care, home health agencies, outpatient clinics, and community-based programs. These services are provided by trained nutrition professionals such as registered dietitians and nutritionists, and include a wide range of interventions:

  • General education on healthy eating and portion control
  • Screening for food insecurity and malnutrition risk
  • Personalized meal planning based on medical conditions
  • Ongoing counseling to encourage sustainable lifestyle changes
  • Reinforcement of dietary advice provided by healthcare teams

Our in-house Registered Dietitian team provides hands-on support through nutritional assessments—screening members for dietary allergies and restrictions—as well as nutritional counseling, which includes education and tailored meal plans based on each member’s chronic condition. Our wellness coordinators assist with member outreach, and our in-house operations and customer support teams ensure seamless coordination of care and communication.

In addition to addressing physical health, proper nutrition is closely tied to cognitive function, mental well-being, and immune system strength. Early identification of nutritional deficiencies, coupled with targeted interventions, can significantly improve patient resilience, reduce complications, and support faster recovery times. These services are essential for both preventing disease and improving quality of life across all age groups and care levels.
